Your visit not only includes a thorough assessment, lab draw and follow up, consistent monitoring of blood pressures, etc., but you are also encouraged to set goals, identify barriers and challenges, and take an active role in monitoring your own conditions. Many tools and resources will be available to you to provide visual reminders that help you manage your health. I offer two service options, Direct Primary Care and Non member cash pay services.
